Required Forms

Once you make an appointment with Be Fit Massage you will be asked to fill out a form or two. The form everyone needs to fill out before massage is the Health History Form. Your personal information will be kept completely private and is never shared with anyone for any reason. By letting me know about your health I can tailor your massage experience to meet your needs.

If you choose to have medical deep tissue massage (mdt) you will need to fill out the Informed Consent to Medical Deep Tissue Massage Therapy form. We can discuss mdt by phone or in person before you sign this form.

Both forms are provided in Adobe Acrobat format (pdf). You will need Adobe Acrobat Reader to view and print these forms. You can download the reader for free.
