Once you make an appointment with Be Fit Massage you will be asked to fill out a form or two. The form everyone needs to fill out before massage is the Health History Form. Your personal information will be kept completely private and is never shared with anyone for any reason. By letting me know about your health I can tailor your massage experience to meet your needs.
If you choose to have medical deep tissue massage (mdt) you will need to fill out the Informed Consent to Medical Deep Tissue Massage Therapy form. We can discuss mdt by phone or in person before you sign this form.
